Buy a bike from one of their approved dealers. Fill in the paperwork and submit it by the deadline. There is a current one and I think we got $200 or $300. Something to watch for is the tax. We qualified for one level of rebate based on the price of the bike, but the receipt shows slightly over $2k and that pushed us up to a higher level of rebate.

It was a pretty straightforward process.

Also, if you have a credit card that extends warranty on items purchased (like the Costco Visa) use that. Batteries are one of those things that could be costly and not user friendly in terms of replacement (depending on the bike). My wife's bike has them integrated into the downtube so having additional warranty is really valuable.
